By scheduling or receiving services, you acknowledge and agree that Neuromuscular Therapy, CranioSacral Therapy, and Massage Therapy are non-medical wellness services intended to support relaxation, pain management, mobility, and nervous system regulation.

You understand that services are not a substitute for medical diagnosis, treatment, or care from a licensed physician. Practitioners do not diagnose conditions, prescribe medications, or perform spinal manipulation.

You acknowledge that, as with any therapeutic service, there are inherent risks, and you voluntarily assume responsibility for your participation in treatment. You agree to provide accurate health information and to inform your practitioner of any medical conditions, injuries, pregnancy, or changes in health status prior to each session.

You understand that you may stop or modify treatment at any time and agree to communicate any discomfort during a session.

By booking or receiving services, you consent to treatment and release NeuroMuscular Pain and Wellness Center from liability to the fullest extent permitted by law.
